Builder fined $38K in death of a worker

HONOLULU - A Hawaii builder must pay nearly $38,000 in citations in the death of a construction worker.

A state investigation found that the foreman and employees failed to install and use fall protection systems while walking and working on a roof of a two-story house in October.

The Hawaii Department of Labor and Industrial Relations says Landmark Pacific Builders Inc. violated five safety and health requirements and has agreed to pay the citations.

Company owner James Chan declined to comment Monday.

Acting Hawaii Occupational Safety and Health Division Administrator Doreen Miyaki says failing to provide fall protection systems and training is consistently the leading cause of preventable workplace fatalities in Hawaii.
