DOH confirms fifth cyclosporiasis case

The Hawaii Department of Health has confirmed the state's fifth case of cyclosporiasis this year in a military member based in Hawaii who had traveled internationally and to another state reporting increased cases.

Officials have not determined where the infection was acquired, but said the risk to the public in Hawaii remains low. The state's 2026 total includes four confirmed cases and one probable case, with no known connection between the two most recent cases.

The department is also investigating a low-level detection of the Cyclospora parasite in wastewater from the Hilo Wastewater Treatment Plant. Officials said the finding alone does not establish community transmission or a connection to any known case.

Cyclosporiasis commonly causes frequent, watery diarrhea and may also cause stomach cramps, nausea, fatigue and weight loss. People experiencing persistent diarrhea should contact a health care provider.
